These terms govern your use of app.claimgotdenied.com, operated by ClaimGotDenied. By using the service, you agree to them.
What this service is, and is not
ClaimGotDenied is a self-help document tool. It analyzes the denial letter you upload and generates a first-level internal appeal letter and evidence checklist based on the information you provide. It is not legal advice and not medical advice. We are not a law firm, not attorneys, not physicians, not patient advocates, and not your insurance company, and we do not represent you before any insurer, plan, or regulator. No attorney-client or provider-patient relationship is created. You review and file your own appeal.
No guaranteed outcome
Whether an appeal succeeds depends on your plan, your medical facts, your documentation, and your insurer’s process. We do not promise, predict, or guarantee that your appeal will be granted, that any claim will be paid, or any other outcome. Classification of your denial and any deadline we display are automated readings of your document and may contain errors, so always verify them against the letter itself.
Your responsibilities
- Upload only documents about your own claim, or one you are legally authorized to act on.
- Review every document before you rely on or submit it, and fill in the placeholders.
- Confirm the appeal deadline and appeals address printed on your own denial letter, because those control, not this tool.
- File your appeal yourself, on time. You are solely responsible for the filing.
- Get medical questions answered by your physician and legal questions by a licensed attorney.
Expert-help referrals
If you request expert help, we pass your contact details to an independent patient advocate or attorney with your consent. They are not our employees or agents; any engagement is between you and them, on their terms.
Payment and refunds
The appeal packet is a one-time purchase of $99. It is a custom digital document delivered to you immediately after payment. If a technical problem on our side prevents delivery, you are entitled to a full refund. If you believe there is a problem with your document, contact us at support@claimgotdenied.com within 14 days and we will correct it or issue a refund at our discretion. Apart from that, because the product is custom and delivered immediately, sales are final.
Acceptable use
Use the service only for genuine appeals of your own (or an authorized person’s) denied claims, provide accurate information, and do not resell, scrape, or attempt to disrupt the service.
Intellectual property
We own the ClaimGotDenied software and templates. The completed appeal documents we generate for you are yours to use for your own appeal.
Disclaimers and limitation of liability
The service is provided “as is” without warranties of any kind. To the maximum extent permitted by law, ClaimGotDenied’s total liability arising out of or relating to the service is limited to the amount you paid us in the $99 transaction at issue. We are not liable for indirect, incidental, or consequential damages, including missed deadlines, denied appeals, or unpaid claims.
Indemnification
You agree to hold ClaimGotDenied harmless from claims arising out of your use of the service or your filings.
Governing law
These terms are governed by the laws of the State of Texas, and any dispute will be handled in the courts located there.
Changes
We may update these terms; the effective date above will change when we do.
